  • Copie de la facture portant obligatoirement la mention acquittée, le cachet de l’Organisme de formation et la signature de l’Organisme de Formation
  • Attestation de présence OU certificat de réalisation OU feuille d’émargement par demi-journée* qui indique les dates de formation, mentionne le nombre d’heures réalisées, comporte la signature, le nom du formateur et ceux du stagiaire
    *les feuilles d’émargement sont obligatoires pour les formations de plus de 100 heures
  • Le cas échéant, l’attestation de demande de remboursement de frais annexe
